CAPTURE recently launched its free web-based repository for finding and sharing
practice-based evidence in health promotion and chronic disease prevention. Now,
we need to populate it with a critical mass of information and real-life learnings about
programs, projects and policy initiatives across Canada.

this form and returning it to us. Once we have uploaded your information to the
Platform, simply verify it online to become eligible to win an iPad 2 (draw December 15,
2011). Each program or intervention you add to the CAPTURE Platform gives you a

Our program is Motiv8 a program that promotes 8 evidence based behavior to get and keep a healthy weight. We have a website, media campaigns and workplace 5 part one hour series that is awareness and skill development based. Healthy Children Healthy Futures is a community developed, peer facilitated, modular based parent education and discussion program for parents of children 0 to 6 years old.
Healthy Children Healthy Futures has been serving families in the Cowichan and in the Port Alberni fRegions for over 16 years. We discuss the 14 Module topics that include themes from Child Safety to Stress Prevention to Daily Living and even to Community Resources.
We are a Practice Based Program and we listen to the feet of our participants to make this the best Parenting group anywhere.
This program has been cooperatively developed by the Aboriginal Friendship Centres in Duncan and in Port Alberni with the support of the Community Action Program fro Children from the Public Health Agency of Canada.
